Client Agreement
Please read the following as an agreement towards our working together.
My approach to healing and personal transformation is holistic, focusing on you as a unique, complex, dynamic being of mind, body and spirit. I offer to serve as a facilitator in your self-initiated process of healing and transformation.
Self-care is extremely important. At all times, self-care is your responsibility. If at any time during a session you are uncomfortable, it is your responsibility to inform me immediately. I recommend that you refrain from alcohol and recreational drug use 48 hours before and after your Vitality Session. Please schedule your appointment during a time where you can give to yourself fully.
My approach is non-judgmental, direct, gentle, and supportive, enabling you to be wherever you are in your transformation process and however you choose to express your beliefs and spirituality. Certain problems, either physical or emotional, may be alleviated. Most people really enjoy having a session; a deep sense of reassurance, a feeling of support, revitalized energy and renewal of personal power are common benefits. Deep mystical experiences can occur, as well as life-change realizations. Sometimes, suppressed emotions or physical tensions may pushed to surface so they can be released, resolved, or given more attention, and this process may create various emotional or physical symptoms. Deep transformation is a process that is intended to create changes in ones life, and those changes can manifest physically, emotionally and spiritually.
You agree that you have read and understood the information above and agree that Hedieh Shariatmadari/Vitality Elevated is not responsible for any individual symptoms that may arise as a result of receiving a Consultation or Energy Transmission. You agree to take personal responsibility for whatever physical or emotional symptoms may arise as part of the transformation process, as well as to take responsibility for seeking medical treatment whenever necessary.
You understand that Hedieh Shariatmadari/Vitality Elevated is neither a medical professional nor a psychotherapist and that she is practicing neither medicine nor psychotherapy. Although she may comment on the nature of body energetics and consciousness in relation to disease and mental health, it is understood that these comments are not intended as advice for any course of action for any medical or mental health issues that you may have.
You understand and agree that Hedieh Shariatmadari’s Consultations do not take the place of medical diagnosis, treatment or evaluations.
Alternative Practitioner; California SB577 Disclaimer: This Legal Disclaimer allows alternative practitioners to practice legally in the State of California. According to the California Health Freedom Act (SB577), the following information must be disclosed to you. Please retrieve the full text of SB577 from the Official California State Legislative Information website. The California Health Freedom Act was signed into law on September 23, 2002. This bill allows full practice of alternative methods by non-licensed individuals, within certain clearly defined parameters. As of January 1, 2003, alternative practitioners are allowed to practice freely in California.
You understand that any payments for sessions are not for any specific results. You agree that you are liable for payment of all scheduled appointment unless 48 hour cancellation notice is given.
Limitations and Liability
In no event shall provider be liable for any cause or claim whatsoever arising of or related to this agreement.
Payment made towards consultations and services offered by Hedieh Shariatmadari & Vitality Elevated are an agreement to these statements and policies.
